SOCIAL WORKER – TEMPORARY ASSIGNMENT

United Staffing Solutions, a leader in the staffing industry has the pleasure of assisting a very well-established non-profit organization who offers healthcare services to the elderly, disabled and chronically ill.

A vehicle is required for field visits.

Salary - $42-$45 per hour DOE

Days/Hours – M-F, 9am-5pm.

Responsibilities for the Social Worker:

• Provide coordination of care and ongoing casework services to members
• Collaborate with hospital clinical team to devise treatment and discharge plans
• Maintain current records and complete comprehensive assessments
• Inform members of how to implement health and welfare resources
• Participate in conferences, team and family meetings, and continuing education programs
• Other duties as assigned

• Master’s Degree in Social Work required
• Must have a valid NYS Social Work License
• Minimum 1 year of experience in a healthcare setting
• Excellent written and verbal communication skills
