United Staffing Solutions, a leader in the healthcare staffing industry, is seeking a dedicated and experienced Senior Social Worker, LMSW to join a team in providing compassionate care and critical support to our community.

Location: New York, NY 10039
Schedule: Tuesday - Saturday, 9:00 AM - 5:00 PM
Salary Range: $70,000 - $80,000/year depending on experience

Key Responsibilities:

• Facilitate hospital discharges and coordinate transitions of care.
• Develop and implement comprehensive social work plans for patients and their families.
• Provide assessments, counseling, and advocacy to address psychosocial needs.
• Collaborate with multidisciplinary teams to ensure continuity and quality of care.
• Maintain accurate and timely documentation of all interactions and interventions.

• Must hold an active LMSW (Licensed Master Social Worker) certification in New York State.
• Proven experience in hospital discharge planning is required.
• Strong interpersonal, organizational, and communication skills.
• Ability to work independently and as part of a team.
