Registered Nurse (RN) – Case Manager

Location: Mount Vernon, NY
Pay: $62/hour (based on experience)
Schedule: Full-Time

We are seeking an experienced Registered Nurse Case Manager to join our healthcare team in Mount Vernon. This full-time temporary position is ideal for an RN with strong clinical judgment, care coordination experience, and a passion for improving patient outcomes.

Responsibilities

• Coordinate and manage patient care plans from admission through discharge
• Assess patient needs and develop individualized care plans
• Collaborate with physicians, nurses, social workers, and interdisciplinary teams
• Facilitate discharge planning and coordinate community resources
• Monitor patient progress and ensure continuity of care
• Maintain accurate documentation and ensure compliance with regulatory standards

• Active New York State RN license
• Minimum 2 years of nursing or case management experience
• Psychiatric nursing experience required
• Current BLS certification through the American Heart Association
• Current ACLS certification
• Current CPIT certification
• Strong communication, organizational, and critical-thinking skills
• Ability to work effectively in a fast-paced healthcare environment
