Registered Nurse Case Manager
Job Description
We are hiring two RN Case Managers for the Santa Fe Indian Health Center. You will coordinate care, manage referrals, and collaborate with healthcare teams to ensure efficient patient services.
Location: Santa Fe, NM - Indian Reservation
Pay Rate: $60-$63/hour
Schedule: Monday to Friday, 8:00 AM - 4:30 PM (30-min lunch)
Contract: 2025-2026
Key Responsibilities:
- Coordinate medical equipment orders and manage patient referrals.
- Collaborate with clinical staff, case managers, and external providers.
- Maintain electronic health records and review care plans.
- Educate patients about care options and advocate for benefits.
Job Requirements
- Active RN license with 3+ years of nursing experience, including 1 year as a Nurse Manager.
- BLS and PALS required (ACLS, NRP, TNCC a plus).
- Familiarity with IHS systems and managed care concepts preferred.
