We are seeking a dedicated and compassionate Registered Nurse Case Manager for a hospital in Webster, TX. In this role, you will be responsible for coordinating patient care, ensuring optimal health outcomes, and providing support throughout the patient's healthcare journey.

Pay: $42/hr with weekly pay

40 hours a week

Duties of the RN Case Manager

• Discharge Planning & Referrals: Coordinate patient discharges to home, community, or SNFs. Manage housing, dialysis, wound vac, and shelter needs.
• Caseload Management: Handle 17-25 patients per shift, collaborating with Social Workers.
• Interdisciplinary Teamwork: Participate in rounds with physicians and team members.
• Independent Work: Work autonomously while coordinating with staff.
• Time Management & Problem Solving: Prioritize tasks and think critically to resolve challenges.
• EMR Documentation: Use Allscripts to manage patient records.
• Preferred Knowledge: Familiarity with CarePort, MCG, and/or InterQual.
• Patient Units: Focus on Med-Surg and Tele units, covering inpatient care.

• Current Registered Nurse (RN) license in Texas.
• Proven experience in patient care within a hospital or acute care setting.
• BLS (Basic Life Support)
• MUST BE A REGISTERED NURSE
• PRI (Patient Review Instrument) Certification
• SCREEN Training Certification
