Job Description
United Staffing Solutions, a leader in the healthcare staffing industry, is seeking a dedicated and experienced Registered Nurse Case Manager in The Bronx, NY. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey.
Pay: $80/hr with weekly pay
Responsibilities of the RN Case Manager
- Conduct comprehensive assessments of patients’ medical and psychosocial needs to develop individualized care plans.
- Handle a caseload of 17-25 patients on floors with 33-40 patients (1 CM & 1 SW) or 12-17 patients on Pilot Floors (1 CM & 2 SWs)
- Must be able to manage at least 20 patients, typically on Med-Surg or Tele Floors following Internal Med and Family Med.
- Provide education and resources to patients and their families regarding health management and available services.
- Maintain accurate documentation of patient interactions in compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams to facilitate effective communication and continuity of care.
- Conduct diagnostic evaluations to identify patient needs and barriers to care.
- Advocate for patients within the healthcare system to ensure they receive appropriate services.
- Stay informed about best practices in case management and participate in ongoing professional development.
Job Requirements
- Current Registered Nurse (RN) license in the state of practice.
- EMR: Allscripts
- Strong understanding of behavioral health issues and critical care practices.
- Familiarity with primary care protocols and clinic operations.
- Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse populations.
- Ability to work collaboratively within a multidisciplinary team setting.
- Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.
- Strong organizational skills with attention to detail and the ability to manage multiple priorities effectively.
- Certification in case management is preferred but not required.
