We are seeking a skilled and compassionate Registered Nurse (RN) specializing in Case Management. In this role, you will work closely with patients and healthcare providers to ensure effective care coordination and efficient use of resources. Position Details:

• Location: Austin, TX
• Contract Length: 13 weeks
• Schedule: Monday through Friday, 8:00 AM - 5:00 PM

Compensation:

• Gross Weekly Pay: $1,800 - $2,040

Responsibilities:

• Develop and manage patient care plans to ensure comprehensive treatment.
• Coordinate with interdisciplinary teams for optimal patient outcomes.
• Monitor patient progress and update care plans as necessary.
• Advocate for patients to access resources and services.
• Maintain accurate and detailed documentation.

• Active RN license in Texas or compact state.
• Prior experience in case management or a related field.
• Excellent organizational and communication skills.
• Ability to work independently and manage multiple tasks.
