Care Coordinator (Case Manager) – RN

We are currently seeking experienced Care Coordinator Registered Nurses to support patient care in collaboration with the nursing team and providers at a clinic located in Sanders, AZ. This role is focused on providing comprehensive case management services in a daytime outpatient setting.

Location: Fort Defiance, AZ – Clinic Setting
Rate: $70/hour (weekly pay)
Schedule: Monday–Friday, 8:00 AM to 5:00 PM
No weekends or holidays required

Key Responsibilities:

• Coordinate care and services for patients across the healthcare continuum
• Collaborate closely with providers, nurses, and other clinical staff
• Monitor and manage patient treatment plans and outcomes
• Ensure continuity of care and proper follow-up
• Facilitate referrals, authorizations, and community resource linkages

• Active RN license (AZ or compact)
• Minimum of 2 years of clinical experience preferred
• Case management experience strongly preferred
• Strong communication and organizational skills
• Ability to work independently and as part of a multidisciplinary team

• Orientation: Offered every other Monday.
