PATIENT CARE COORDINATOR

You will be reporting directly to the Vice President and Chief Medical Officer, and Clinical Projects Manager

Salary - $100,000

· Excellent Benefits Package.

Hours/Days – M-F from 8am-4pm or 9am-5pm

Responsibilities for the Patient Care Coordinator:

• Work closely with providers by creating care plans
• Help patients in establishing treatment goals
• Counsel patients on individual care plans
• Offer outreach to high-risk patients
• Participate in team meetings
• Ensure patient scheduling by working with the Benefits Counselor and Outreach staff
• Perform other duties as assigned

• Bachelor’s Degree
• Must have valid NYS RN License
• 2 years of clinical and administrative experience in a similar environment
• Prior clinical experience preferred
• Knowledge of EMR software
• Ability to form and maintain relationships with families and staff
• Must have excellent written and verbal communication skills
• Must be Covid and flu vaccinated
