Job Description

Licensed Occupational Therapist Consultant (Part-Time)

We are seeking a Licensed Occupational Therapist Consultant to join a city-funded home modification initiative focused on improving safety and accessibility for older adults living independently. In this role, you will conduct in-home assessments across New York City, identify fall risks, recommend home modifications and adaptive equipment, and collaborate with contractors and program staff to ensure successful implementation.

Pay Rate: $60–$65/hour depending on experience
Location: New York City – All Five Boroughs
Job Type: Contract | Part-Time | Hybrid (In-Person & Remote)

Key Responsibilities:

  • Conduct in-person home visits throughout NYC to evaluate safety, falls risk, and functional needs
  • Recommend home modifications, repairs, and durable medical equipment (DME) to reduce fall risk
  • Collaborate with project staff and contractors on service delivery and timeline management
  • Provide follow-up training to clients on new equipment or modifications
  • Input data into program systems and help prepare reports
  • Participate in outreach efforts and coordinate with referral sources
  • Support the development of sustainable practices and resource materials

Job Requirements

  • Licensed Occupational Therapist in New York State
  • Minimum 3 years of full-time OT experience, including 1 year in an educational setting
  • Strong background working with older adult or medically complex populations
  • Experience in home-based assessments and DME recommendations
  • Ability to travel to client homes across all five boroughs
  • Must have reliable internet access and laptop/computer with video conferencing capabilities
  • Excellent communication and documentation skills

Preferred:

  • Bilingual in Spanish, Russian or Chinese
  • Experience working with low-income or underserved populations

Schedule & Term:

  • Flexible, part-time hours based on program need
  • Estimated contract period: August 2025 through March 2028
