Full-Time

Pay: $37–$40/hour depending on experience

Key Responsibilities:

• Conduct assessments for newly admitted residents and their families to identify treatment needs and ongoing concerns.
• Represent the psychosocial and emotional needs of residents and families during interdisciplinary team meetings.
• Collaborate with medical, nursing, and other professional staff to ensure integrated, high-quality care.
• Participate in meetings related to resident and family issues, offering solutions and support.
• Ensure all services meet federal, state, and facility standards and protocols.
• Stay informed of best practices by attending in-service training and professional development sessions.
• Serve as a liaison between residents, families, and staff to resolve concerns and support well-being.
• Assist in discharge planning by connecting residents and families with appropriate community resources.
• Support additional social service-related duties as needed.

• Licensed Master Social Worker (LMSW) – New York State required.
• Minimum of 2 years’ experience in long-term care, mental health, or related healthcare settings.
• Demonstrated experience with HIV-positive populations and/or adult mental health strongly preferred.
• Excellent verbal and written communication skills.
• Collaborative mindset and ability to thrive in an interdisciplinary team environment.
• Bilingual abilities are a plus.
• Strong interpersonal skills and a genuine passion for helping others.
