We are seeking a dedicated Licensed Practical Nurse (LPN) to provide mobile health services in Ridgewood, NY. This role involves delivering direct patient care in patients’ homes while working under the supervision of an Advanced Practice Provider (APP). The ideal candidate is compassionate, culturally sensitive, and experienced in both adult and pediatric care.

Responsibilities:

• Provide assessments and directed treatment for adult and pediatric patients at their homes.
• Document patient care accurately in compliance with state regulations and EMR standards.
• Practice under synchronous telemedicine support as required.
• Serve as a conduit for medical treatment for patients facing barriers to traditional healthcare.
• Participate in continuing education and adhere to clinical and safety protocols.
• Lead patient interactions with a patient-centered approach, including discussions on health management, medication, and goals of care.
• Perform other related duties as needed.

• Active New York State LPN license (unrestricted).
• Minimum 2 years of experience as a Licensed Practical Nurse (up to 20 years acceptable).
• Current BLS certification (AHA required).
• Experience in Emergency Department, Urgent Care, Internal Medicine, Critical Care, Family Practice, or other relevant areas preferred.
• Pediatric clinical experience required.
• Phlebotomy experience required.
• Active driver’s license for 2+ years required.
• Ability to lift 50–60 lbs.
• Spanish fluency preferred.
