Case Management RN
Location: Portsmouth, NH 03801
Shift: 8:00 AM – 4:30 PM | 13-week contract
Pay: $2,400/week
Responsibilities:
- Coordinate patient care across departments and providers
- Perform utilization reviews and discharge planning
- Advocate for patient needs and ensure continuity of care
- Maintain accurate case management documentation
Job Requirements
- Active compact RN license
- At least 2 years of hospital case management experience required
- Strong organizational and communication skills
