Case Management RN
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our case management team at a hospital in Phoenix. The successful candidate will oversee patient care coordination, work closely with a multidisciplinary team, and ensure smooth transitions through various levels of care.
Shift Details:
- 8-hour AM shift
- Day shift
Pay: $2255/week gross
Start Date: June 23rd
Location: Phoenix, AZ 85024
Key Responsibilities:
- Develop and implement individualized case management plans
- Coordinate with healthcare professionals to optimize patient care
- Monitor patient progress, communicate with families, and adjust plans as necessary
- Maintain accurate and timely documentation
Job Requirements
- Valid RN license
- Experience in case management or related field preferred
- Excellent communication and organizational skills
- Ability to work effectively in a fast-paced environment
