Case Manager – Danville, VA – $2,000 Weekly
Responsibilities:
- Assess patient needs and create individualized care plans.
- Monitor patient progress and coordinate with healthcare providers.
- Ensure compliance with all healthcare regulations and policies.
- Educate patients and caregivers on care plans and available resources.
- Maintain detailed and accurate medical records.
Job Requirements
- Active RN license in the state of Virginia (or compact state).
- At least 1–2 years of case management or related nursing experience.
- BLS certification required.
- Strong problem-solving and communication skills.
