We are seeking Case Managers to provide community-based support to clients throughout Westchester County. The primary location is in Yonkers, with field work in Yonkers, Mount Vernon, and New Rochelle.

The Case Manager will meet directly with clients, coordinate services and resources, maintain case documentation, and enter timely notes into electronic record and data management systems.

Responsibilities

• Meet directly with clients to assess needs and provide ongoing case management support
• Connect clients with appropriate services, programs, housing resources, and community resources
• Develop and monitor individualized service plans
• Maintain accurate and timely case notes in the Electronic Health Record (EHR)
• Enter and maintain client information using web-based data management systems
• Monitor client progress and follow up on identified needs
• Coordinate services with providers and other members of the care team
• Travel to assigned sites and conduct field work in Yonkers, Mount Vernon, and New Rochelle
• Maintain professional, accurate, and confidential client documentation

• Associate Degree required; Bachelor’s Degree preferred
• 1–2 years of similar experience in housing/homelessness prevention and/or relevant social service case management
• Experience using web-based data management systems, including HMIS
• Strong computer skills, including Microsoft Word and Excel
• Excellent written and verbal communication skills
• Ability to work effectively both independently and as part of a team
• Bilingual Spanish/English is a plus
• Valid driver’s license and access to an automobile is a plus
• Must be comfortable traveling between Yonkers, Mount Vernon, and New Rochelle

Pay: $28/hour | Monday–Friday, 9:00 AM–5:00 PM
