Job Description
United Staffing Solutions, a leader in the healthcare staffing industry, is seeking Community Habilitation Workers in Queens (Jamaica area). You will be responsible for delivering high-quality care to assigned patients.
Monday through Friday Morning role.
Pay: $17.00 - $18.00 per hour DOE
Essential Duties and Responsibilities include the following.
- Responsible for maintaining updated documents in individuals’ general binders.
- Complete Monthly Progress Notes. Complete monthly case notes in the “Case Note” section of the individuals’ general binders.
- Review with Care Manager, advocate and individual the Staff Action Plan every 6 months and complete Staff Action Plan based on goals identified at review every 6 months.
- Address Life Plan goals and ensure that monthly progress notes and checklists match staff actions. Address Staff Action Plan goals and progress as described in the Life Plan and outlined in the Staff Action Plan, and assure the checklists reflect that services were rendered as described in the Staff Action Plan and that progress from those services are mentioned in the monthly progress notes.
- Coordinate and attend all individuals’ Life Plan, Staff Action Plan and/or Special Team meetings. Participate in establishing new goals for individuals.
- Conduct groups/activities in a group setting for individuals with Intellectual Disabilities at Gateway and in the field.
- Supervise individuals during field trips to work sites/report all untoward events/incidents to supervisor in a timely manner, as well as to the New York State Justice Center for the Protection of People with Special Needs.
- Sign Checklists to attest to groups conducted and individuals who attended and ensure that Training Specialists in their group rooms do the same.
- Ensure that Sign-In sheets are properly competed and stored.

- Education and/or Experience Associate’s or Bachelor's degree (B. A.) in Health or Human Services or related field from accredited College or university Language Skills Ability to read, analyze, and interpret general documents related to Individuals’ care.
- Ability to write monthly progress notes and case notes that accurately reflect individuals’ progress as related to their Group Day Habilitation Plans.
- Computer Skills To perform this job successfully, an individual should have knowledge of Word Processing software.
- Other Qualifications Valid NYS Driver’s License in good standing Motivation to work with the Developmentally Disabled population
